Your Next Appointment
Whether you're a new or an established patient, Mercer Medicine strives to provide an exceptional patient experience. To help your visit go as smoothly as possible, we encourage you to come prepared.
New Patients
- Complete your new patient paperwork before you arrive.
- Bring the following to your first appointment.
- A bag with your current medications (prescription and over-the-counter) and any dietary supplements you are currently using
- Your current driver's license and/or photo ID
- Your current insurance ID card
- Your office visit copay or deductible
- Arrive at least 30 minutes before your scheduled appointment. Your appointment time indicates the time your doctor is scheduled to see you. You'll need to allow extra time to park, check in with our front desk, settle any copays, and complete necessary forms. Some providers may require you to reschedule your appointment if you do not arrive with enough time to complete these required tasks.
Established Patients
- Please arrive 15 minutes before your scheduled appointment. Your appointment time indicates the time your doctor is scheduled to see you. You'll need to allow extra time to park, check in with our front desk, settle any copays or balances due, and complete necessary paperwork. Some providers may require you to reschedule your appointment if you do not arrive with enough time to complete these required tasks.

Patient Forms
- Mercer Medicine New Patient Paperwork - Complete your new patient paperwork before you arrive.
- Notice of Privacy Practices (PDF) - Describes how health information about you (as a patient of this Care Center) may be used and disclosed, and how you can get access to your individually identifiable health information. Please review this notice carefully.
- Authorization for Release of Medical Information (PDF) - Allows patients to authorize the disclosure of their health information to a designated individual, company, agency, or facility.
- Authorization and Consent for Treatment (PDF) - All patients must provide their consent for treatment, communications (calls, emails, and text messaging), and agreement of financial responsibility. Autorización y Consentimiento Para el Tratamiento
- Preferred Contacts (PDF) - Patients are encouraged to complete and return the Preferred Contacts Form but it is not required. Contactos Preferidos
- Financial Policy (PDF) - This form advises patients of their complete financial responsibility for all medical services received without regard to insurance eligibility or coverage determinations.

Insurance
Mercer Medicine accepts most major insurance plans. To ensure your plan is in-network with Mercer Medicine, we recommend you confirm coverage directly with your insurance company before your appointment. If questions about your plan or if you are not covered by health insurance, our service team can provide information and assistance. Call 478-301-5111.
Payments
At Mercer Medicine, payment is expected at the time of service. We will bill your insurance company for covered services, but you are responsible for co-payments, deductibles, and non-covered services when you check in for your appointment.
All Mercer Medicine locations accept the following forms of payment:
- Cash
- Flexible Spending Account (FSA) cards
- Money Orders
- Personal Check
- Visa or Mastercard debit and/or credit cards
How to pay your bills online
If your appointment was prior to April 29, 2024, view your records and settle your bills via your Healow Portal. For any appointments or procedures scheduled after April 29, 2024, access them through our newPrivia Health Patient Portal.
If you have questions about your bill, please call 888-774-8428. For your convenience, you may also view and pay your bill online through our patient portal. You can easily access this secure patient portal here.
Policies
Cancellation and No-Show Policy
Failure to cancel appointments within 24 hours will result in a no-show status. Effective June 1, 2024, a $25 fee will be imposed for all no-shows. This fee cannot be covered by insurance.
Lab Policy
Mercer Medicine has a contract with Quest Diagnostics. If your insurance requires you to use a different lab, you must inform the staff at the time of your appointment. We recommend you verify your coverage directly with your insurance company before your appointment.
Privacy Policy
We are committed to treating you and your protected health information responsibly. Please refer to our complete HIPAA Notice of Privacy Practices to learn about the personal health information we collect, and how and when we use or disclose that information.
Patient Portal
Mercer Medicine's patient portal allows you a simple, secure way to manage your health information. Through the portal, you can manage appointments, send a message to your provider, view test results, request a prescription refill, and make payments online.
